HER PNÖMOTORAKS, PRİMER SPONTAN PNÖMOTORAKS DEĞİLDİR!

Pnömotoraks, plevral boş lukta serbest hava bulunması halidir ki; en sık olarak primer spontan pnömotoraks şeklinde karşımıza çıkar. Sekonder spontan pnömotorakslarda ise; pnömotoraks zemininde sorunlu ya da patolojik bir akciğer yatmaktadır. En sık sebep ise KOAH'tır. Ancak birçok sekonder pnömotoraks, altta yatan akciğer patolojisinin çok belirgin olmadığı hallerde, primer spontan pnömotoraks olarak kabul edilmekte; temeldeki akciğer patolojisi atlanabilmektedir. Yirmisekiz yaşında erkek hasta, sabah uykusundan uyandıran göğüs ağrısı şikayeti ile acil servise başvurdu. Dinlemekle sağda solunum sesleri sola göre belirgin azalmıştı. Çekilen PA akciğer grafisinde, "sağda parsiyel lateral pnömotoraks hattı" izlendi. On paket yılı sigara içicisiydi. Kan sayımı ve rutin biyokimyasal tetkikleri olağan bulundu. Sedimentasyon 21 mm/saat idi. Hasta bu halde primer spontan pnömotoraks kabul edilerek tüp torakostomi yapıldı ve kapalı su altı drenajına bağlandı. Tüp torakostomi sonrası çekilen toraks bilgisayarlı tomografide, her iki akciğer apeksinde izlenen büllöz alanlar dışında patolojik görünüm izlenmedi. Uzamış hava kaçağı gelişmesi üzerine, standart pnömotoraks cerrahisi yapıldı. Postoperatif 7. günde şifa ile taburcu edildi. Ancak rezeke edilen materyalin histopatolojik inceleme sonucu "kronik granülomat öz inflamasyon" olarak rapor edildi. Dış merkezde yapılan immünhistokimyasal değerlendirmeler sonucunda, histopatolojik veriler ve S100 pozitifliğine dayanarak "Langerhans cell histisitozis (Eozinofilik Granülom)" tanısı konuldu. Olgumuzu bu halde, nadir görülen bir sekonder spontan pnömotoraks, etyolojisine sahip olması nedeniyle sunuyoruz.

Pneumothorax is a circumstance of air leak's presence at the pleural space that it mostly occurs as a primary spontaneous pneumothorax. It's seen 7.4/100.000 at men and 1.2/100.000 at women in a year. In general, these patients are young adults. At secondary spontaneous pneumothorax, the problem is a pathologic lung on pneumothorax base. The most common reason is COPD. And generally, it's seen at the elder patients. Nonetheless; a good deal of seconder pneumothorax is accepted as a primary spontaneous pneumothorax when the lung pathology can be dismissed on condition that the lung pathology is subclinical. The male patient at the age of 28, consulted the emergency servise for the chest pain which awakes from sleep.In his posterior anterior chest x-ray, it is followed that at right partial lateral pneumothorax line. He is a current smoker for 10 years. The sedimentation rate has been 21 mm/h. The patient was accepted as a primary sponta neous pneumothorax; thereby, he was treated by tube thoracostomy and he was bound to closed underwater chest drainage. On the computerized thorax tomography which was taken after the tube thoracostomy, we have seen the bullous areas in the apex of the both lungs. And there was no other pathology. After the development of extended air leak, it was operated standardized pneumothorax surgery. Patient was discharged at the postoperative 7th day. However, the histopathologic examination of resected material was reported "eosinophilic granuloma". Langerhans cell histiositozis diagnosed with immucytochemical evaluation and etholoogy of S100 positivity. We presented the case because it has a none spontan pneumothorax.
