Nefes darlığı şikayeti olan ve dış merkezde sağ akciğer üst lob apikalde kitle saptanan 61 yaşında erkek hasta, yapılan tetkiklerden sonuç alınamaması ve şikayetlerinin geçmemesi üzerine başvurdu. TTİİAB ile akciğer adenokarsinomu tanısı konuldu. Altı kür karboplatin - pemetreksed kemoterapisi verildi. Karın ağrısı ve ishal şikayeti ile başvuran hastada PET-BT’de; batın ön duvarı komşuluğunda peritoneal yağlı dokuda 7.8 cm uzun akslı patolojik 18-FDG tutulumu saptandı. Alınan biyopsi örneği primer kitleden alınan örnekte olduğu gibi CK-7 ve TTF-1 (+), CK-20 (-) idi. Akciğer adenokarsinomu metastazı olarak değerlendirildi. Progresyon düşünülerek ikinci hat dosetaksel tedavisine geçilen hastaya 10 gün palyatif radyoterapi uygulandı. Periton metastazı akciğer kanserlerinde çok nadir görülen bir metastazdır ve genellikle semptomsuz olduğundan tanısını koymak oldukça güçtür. Ancak karın ağrısı, ishal veya kabızlığın yanında intestinal perforasyon gibi ciddi komplikasyonlara neden olabilir. Tedavi çoğunlukla palyatiftir. Cerrahi ve kemoterapinin yanında radyoterapi de şiddetli semptomları olan hastalarda palyatif tedavi seçeneği olarak düşünülebilir. Periton metastazı kötü prognoz göstergesidir ve ciddi mortalite ve morbiditeye neden olmaktadır.

Lung adenocarcinoma was diagnosed by TTFNAB. 6 cures of carboplatin, pemetrexed was given. The patient admitted with stomach ache and diarrhea. In PET-BT; peritoneal involvement was detected in the peritoneal fatty tissue adjacent to the newly determined anterior abdominal wall, with located 7.8 cm long axis. CK-7 and TTF-1 (+) and CK-20 (-) were seen in the biopsy specimen taken from that region as seen in the specimen taken from the primary mass. It was considered as lung adenocarcinoma metastasis. 10 days palliative radiotherapy was planned for the patient, who still had complaints after second line docetaxel treatment. Peritoneal metastasis is a very rare metastasis in lung cancer. Immunostaining of pathologic specimens provides a differential diagnosis of metastases caused by the primary tumor or other organs. It is very difficult to diagnose the peritoneal metastasis since peritoneal metastasis of the lung carcinoma is usually asymptomatic. However, it may cause serious complications such as stomach ache, diarrhea or constipation as well as intestinal perforation. Treatment is mostly palliative. Besides surgery and chemotherapy, radiotherapy can be considered as a palliative treatment option for patients with severe symptoms. Peritoneal metastasis is an indicator of poor prognostic and may cause severe mortality and morbidity.
